2012-2013 ATHLETIC PHYSICALS
St. Bernards Spports medicine is proud to provide this year's physicals for our covered schools. We are excited to announce the continued partnership of St. Bernards Sports Medicine and Healthcare Medical Group (HMG) in an effort to provide the most comprehensive athletic physical possible. The physicals will be held SUNDAY- APRIL 22 during the HMG Health & Fitness Expo at teh Convocation Center. Athletes and their parents need to enter the BLUE ENTRANCE and you will see the physicals registration tables of your respective school as you enter the arena. Please read the instructions below to assit you in helping make our event a success:
SCHOOL TIMES: (male and female)
Bay 11:00 am
Brookland 11:30 am
Nettleton 12:00 pm
Valley View 1:00 pm Westside 1:45 pm
Jonesboro 2:45 pm
To ensure proper flow, please arrive at your designated time slot. (unless you are uable)
IMPORTANT:
1. Parents- you ust sign and date HISTORY FORM, HEALTHY ATHLETE BLOOD SCREENING and HIPPA SECTION to ensure your child receives the entire physical.
2. The HIPPA section ensures confidentiality of information collected during the physicals and management during the school year by the Certified Athletic Trainer and medical staff. the Blod Screening is optional.
3. FORM must be returned to Coaches by March 30th.
4. Waer appropriate clothig- wind pants or gym shorts. No skirts or tight blue jeans. Sport bra recommended for females under their shirts.
5. Coaches are encouraged to be present to help with unsigned forms and crowd control.
6. NO MAKE UP DATE!
